How to apply to change the name in a BSNL landline in bangalore incase of death ?

This is for the name transfer in case of death

1 - Fill the form with basic information .
2 - The form needs affidavit
That i will not hold bsnl responsible for any loss
This is the most painful part
Many Affidavits

3 - it needs reference signature
your friends or colleagues or neighbors  should give consent that they know you.
At least 3 of them.
4 - Notary signature on the affidavit. 
You can get from the government doctor. 
5 - Death certificate original or attested or notarized.
6 - Forms has the Affidavit at the end

Lot of procedure for name change
This will take approx 1 month to get it done.
